Medication Administration Records (MAR charts) are a vital component of healthcare documentation. They ensure that every dose of medication is recorded accurately, providing a clear audit trail of who administered the medication and when. This process is essential for patient safety, compliance with regulations, and maintaining trust in healthcare practices.

Signatures serve as a critical safeguard, confirming accountability and accuracy in medication administration. They help prevent errors, support legal and regulatory requirements, and protect both patients and healthcare professionals. Removing signatures would compromise these safeguards, making it impossible to verify who administered medication and at what time, which could lead to serious risks for patient safety and record integrity. Due to this, signatures can't be removed from MAR charts.
